What this template is
The Health Insurance Card template stores payer card details—member ID, group number, plan type, and pharmacy routing numbers. Enable it under Settings → Category templates → Health & Medical.
When to use it (and when not to)
Good fits
- Front-desk card with member and group IDs
- Family cards with separate member IDs per dependent
- Travel medical card alongside Medical Provider
Use something else instead
Everyday workflows
- Enable Health Insurance Card under Enable category template packs → Health & Medical.
- One item per card or dependent; link the payer phone for quick dial after unlock.
- Share insurer and plan type via selective disclosure—member ID stays hard-blocked.

What you need
Steps
Enable the pack
- Unlock your vault and open Settings → General → Category templates (
https://se.nt2.me/settings/general/templates).
- Under Health & Medical, toggle Health Insurance Card On.
Create a card item
- Open
https://se.nt2.me/assets and select Add asset → Health insurance card.
- Enter a title and fill insurer, member ID, and group number.
- Add plan type, BIN/PCN, and plan end date from the physical card.
- Save. NT² may show a duplicate hint when member IDs match an existing item.
Front-desk or pharmacy lookup
- Open the item from the asset list.
- Use Safe presentation mode to show insurer and plan type without member ID on a shared screen.
- Copy member ID or group number only when staff need to type them—clipboard auto-clears after 30 seconds.
Share with a caregiver
- Use selective disclosure to include insurer and plan type.
- Member ID, group number, BIN, and PCN stay hard-blocked in share flows by default.
Tips and common mistakes
- Not the same as Insurance template—policy-level coverage belongs in Insurance.
- Portal logins in Credential—payer website passwords are not member IDs.
- One item per card or dependent—split family cards instead of one Secure Note block.
- Product boundary—NT² stores card metadata only; it does not verify eligibility or file claims.
